HIPAA Policy<\/b><\/p>

This HIPAA Policy describes Our policies and procedures on the collection, use and disclosure of Your information as it relates to Protected Health Information (PHI)<\/span><\/p>

1.1. Treat Mental Health Utah may use and disclose PHI about an individual for the purposes of treatment, payment, and healthcare operations without obtaining the individual\u2019s authorization.<\/span><\/p>

1.2. In the event that a client falls ill and requires hospitalization, Treat Mental Health Utah may disclose relevant PHI to healthcare providers involved in the client\u2019s care to ensure appropriate treatment.<\/span><\/p>

1.3. If someone poses a potential threat to themselves or others, Treat Mental Health Utah may disclose necessary PHI to the appropriate authorities or individuals who can mitigate the threat, in accordance with applicable laws and regulations.<\/span><\/p>

Individual\u2019s Rights and Exercising Those Rights<\/b><\/p>

2.1. Individuals have the right to inspect and obtain a copy of their own PHI maintained by Treat Mental Health Utah, with certain limited exceptions.<\/span><\/p>

2.2. Individuals have the right to request corrections or amendments to their PHI if they believe it to be inaccurate or incomplete, provided that the request meets specific criteria outlined by applicable regulations.<\/span><\/p>

2.3. Individuals have the right to request restrictions on the use or disclosure of their PHI for treatment, payment, or healthcare operations, and Treat Mental Health Utah will consider such requests, except where prohibited by law.<\/span><\/p>

2.4. Individuals have the right to request confidential communications of their PHI, such as through a specific email address or mailing address, and Treat Mental Health Utah\u00a0 will accommodate reasonable requests.<\/span><\/p>

2.5. Individuals have the right to obtain an accounting of certain disclosures of their PHI made by Treat Mental Health Utah, except for disclosures related to treatment, payment, healthcare operations, or other exceptions as defined by applicable regulations.<\/span><\/p>

2.6. Individuals have the right to receive a notice of privacy practices that explains their rights and how Treat Mental Health Utah uses and discloses PHI.<\/span><\/p>

Legal Duties of the Covered Entity<\/b><\/p>

3.1. Treat Mental Health Utah is committed to maintaining the privacy and security of PHI in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and other applicable laws and regulations.<\/span><\/p>

3.2. Treat Mental Health Utah will use appropriate administrative, technical, and physical safeguards to protect the confidentiality, integrity, and availability of PHI.<\/span><\/p>
